Making a Will is something we all think about at significant stages of our lives but how many of us would think about creating an Advance Care Plan?
A key focus of the Practice Development Team’s work is Advance Care Planning. None of us know exactly how our health and abilities might change in the future and even though we cannot see into the future, we might have some ideas about the sort of help and care we would or would not want.
Planning ahead, using an Advance Care Plan, gives you the opportunity to think about, talk about and write down your wishes, preferences and priorities for your care. This will give everyone a clear idea of the things that are important to you, if there comes a time when you cannot express your wishes and preferences yourself.
The Practice Development Team teaches many different healthcare professionals about Advance Care Planning including nurses, doctors, carers and students. Amanda and her team would also like to talk to the public about Advance Care Planning, to try to spread the word about this really important topic.
Many people welcome the opportunity to take control of their care at a really difficult time through completing an Advance Care Plan:
