Referrals must be from; GP’s Hospital consultants and their teams (in liaison with the Hospital Palliative Care Team), Community Nursing and other providers of domiciliary nursing care.

We cannot accept referrals for patients with chronic stable disease, or those with a life expectancy of several years, patients with chronic pain problems not associated with a progressive terminal disease, or patients whose needs are principally psychological, and need a specialist psychiatric referral, whether or not they have declined such help.
We accept referrals for patients with complex symptoms that are not improving despite treatment. The symptoms may be a combination of physical, such as pain or nausea, emotional or spiritual. Most patients will stay less than two weeks before discharge, by which time symptoms are usually under control.
Cynthia Spencer Hospice accept referrals for care in the last days of life where a patient has expressed a wish to come to the Hospice.
There are three teams that provide care to support patients being able to stay at home.
This service operates Monday to Friday, from 9am to 5pm. The team have specialist knowledge and qualifications in cancer and palliative care. They can provide advice about an illness, the pain control and management of difficult symptoms, emotional support for patients, as well as plan for future care.
The emphasis of the Physiotherapy team is to maximise a patients’ ability to function, to help them to be independent and adapt to their condition.
A large part of the Physiotherapists role is to provide non-pharmacological management of symptoms i.e. fatigue, breathlessness, sweats or pain
This service operates Monday to Friday 9am to 5pm.
The team assists patients and the impact of their illness on their daily life. They work with the patient and carer and provide specialist equipment should they need it to support them in their daily life.
